OIL CHANGE REBATE INSTRUCTIONS & FORM
Provided to you as an unprecedented “life of membership” enrollment
bonus, you are entitled to reimbursement for the cost of up to four (4) oil
and filter changes per year for the automobile you have on file with your membership
profile. The maximum reimbursement for a single oil & filter change is
$30 or up to $120 per year of membership.
Instructions for Oil Change Rebate:
- You are eligible to receive a cash rebate of a single oil & filter
change every 90 days or 3,000 miles whichever is the longer period.
- You may have your oil changed anywhere you choose. However, rebates per
oil and filter change will be a maximum of $30 or the lowest discount
price amount available from oil change vendors who may be listed in our
national network of oil change providers within a 15 mile radius from your
home, or, if you are traveling the lowest discount price available in the
city in which you select to have your oil changed where AMAC has an oil change
provider. If there are no lower priced oil change providers from the vendor
list within 15 miles from your home or within the city you have traveled
to you will receive a rebate for the cost of your oil change or $30 whichever
is the lower amount.
- Rebates are for oil and filter changes only and do not include any other
services or products like air filters, windshield wipers, disposal fees,
taxes or any other product or service you may decide to purchase at the time
of your cars oil change.
- Your request for an Oil Change Rebate must occur within the first 30
day period immediately following every 90 days or 3,000 miles whichever
is the longer period.
- ELIGIBILITY PERIODS DO NOT CARRY FORWARD. Failure to follow the
instructions for a rebate as outlined in this form will result in the loss
of your benefit for only that specific period of eligibility.
- Only the vehicle listed on your AMAC Vehicle Enrollment form are eligible
for Oil Change Rebates.
- To receive your rebate click on the “Print Oil Change Rebate Form” button
below then fill out and mail the form along with a copy of your oil change
receipt from a commercially licensed oil change vendor that list the cost
of the oil and filter change as well as the vehicle for which an oil change
was performed.
